Before your visit
- Bring your insurance card(s) and a photo ID.
- If you have an HMO, get a referral from your primary care doctor first.
- If you have more than one plan (for example Medicare plus a supplement), bring both cards.
- Check your plan for copays, deductibles, or prior authorization rules for testing.
